Your purchase of the Foundational Assessment was successful.  

Before we can schedule your appointment, you need to complete the following steps:

1. Fill out the patient history and health questionnaire in the Living Matrix
2. Log in to our Electronic Medical Record for communication and to retrieve your lab requisition.
3. Go get your blood drawn at Quest, Labcorp or another lab location near you. 
4. Then we will contact you to schedule your Foundational Assessment appointment.
